Vioxx (Rofecoxib)

Purpose of Medication and Method of Action

Vioxx is among the first of a new class of antiinflammatory drugs approved for use in both osteo- and rheumatoid arthritis. It acts by inhibiting the enzyme cyclooxygenase-2 (COX-2), a key step in producing prostaglandins. Prostaglandins are one of the mediators of the inflammatory response.

Side Effects

Abdominal pain, dyspepsia, and headache are among the most common. Although this class of drugs may produce less gastrointestinal bleeding than the NSAIDS (ibuprofen and naproxen) it still may occur and if severe gastrointestinal pain should occur or tarry stools, please contact your physician immediately.

Drug Interactions

Vioxx may decrease the effectiveness of ACE inhibitors (Example Accupril) and diuretics such as hydrochlorothiazide (HCTZ)which are antihypertensives. Fluconazole (Diflucan) use will increase blood levels of Vioxx by two-fold and a Vioxx dose reduction may be advisable.

Cautions and Contraindications

Usual Dose

Osteoarthritis - 12.5mg - 25 mg daily.
