Rezulin (Troglitazone)
Purpose of Medication and Method of Action
Rezulin is indicated as monotherapy for the treatment of maturity onset (Type 2) diabetes. Rezulin acts by improving the sensitivity of cells to the effect of circulating insulin on liver, muscle and fat cells. It is of particular value in overcoming the insulin resistance characteristic of Type 2 diabetes. Avendia is the second drug in this class of agents.
Side Effects
Among the more common side effects are: headache, edema (swelling of ankles) due to increased volume of blood, and anemia (decreased red blood cells). Although a rare reaction, severe liver damage has occurred with Rezulin and liver function studies should be performed every 2 months for the first year of therapy.
Drug Interactions
Rezulin can decrease the blood levels of oral contraceptives thus requiring a higher dose of the contraceptive or the use of alternative methods of birth control.
Cautions and Contraindications
Avendia should be used with caution in presence of edema. Blood volume expansion due to Avendia may worsen existing heart failure.
Usual Dose
The usual starting dose is 400 mg once daily increasing if necessary to 600 mg after one month of therapy.
