Mirapex (Pramipexole)
Purpose of Medication and Method of Action
Mirapex is used to treat the symptoms of Parkinson's disease. It appears to act by stimulating the domamine receptors in the brain striatum - the area where a lack of sufficient dopamine may be the reason for some of the symptoms of Parkinsonism.
Side Effects
The most common side effects are: weakness (14%), nausea (28%), constipation (14%), dizziness (25%), drowsiness (22%), and difficultly sleeping (17%). Occasionally, postural hypotension may occur (low blood pressure upon standing) - if this is a problem, do not get up suddenly but dangle legs over bed for a few minutes before arising.
Drug Interactions
The ulcer medication Cimetadine should be avoided as it will increase the side effects of Mirapex. Dopamine antagonists such as the Phenothiazines (Phenergan, Thorazine) and Metoclopramide will antagonize the effects of Mirapex.
Cautions and Contraindications
When used with levodopa, any preexisting dyskinesia (involuntary movements) may be worsened. A lowering of levodopa dose will resolve.
Usual Dose
The initial dose is 0.125 mg three times a day increasing over 7 weeks to a maximum of 1.5 mg three times a day.
