Mexitil (Mexiletine)
Purpose of Medication and Method of Action
Mexitil is a drug used to treat abnormal heart rhythms, particularly those originating from the ventricle. The mechanism of action is similar to the local anesthetics which inhibit inward sodium current and decrease the effective refractory period in the fibers conducting the electrical impulse from the atria to the ventricle.
Side Effects
The most common side effect is gastrointestinal distress (41%), lightheadedness (10%), tremor (12%) and difficulties in coordination (10%).
Drug Interactions
Mexitil may increase the blood levels (and subsequent toxicity) of theophylline products.
Cautions and Contraindications
Mexitil should not be used in the presence of heart block unless a pacemaker is utilized.
Usual Dose
The loading dose (if necessary) is 400 mg followed by 200-300 mg every eight hours.
