Lotronex (Alosetron)

Purpose of Medication and Method of Action

Lotronex is indicated for the treatment of irritable bowel syndrome (IBS) in women whose predominant bowel symptom is diarrhea. The safety and effectiveness in men has not been established. Lotronex acts by blocking the 5-HT3 receptor on neurons located throughout the gastrointestinal tract. Blockage of these receptors reduces the intestinal hyperreactivity and the accompanying symptoms of abdominal pain and spasm characteristic of IBS. Approximately 60% of those with IBS taking Lotronex experienced adequate relief of their symptoms compared with 40% taking placebo.

Side Effects

The most common side effects are: constipation mild to moderate (25-30%), nausea (7%), abdominal discomfort (5%),and pain (5%)

Drug Interactions

Lotronex is extensively metabolized in the body. Thus, it has the potential of both affecting the metabolism of other drugs as well as being affected itself. Because it is a new drug most possible interactions have not been studied. Thus, if taking a medication with potentially severe side effects such as warfarin, you should have your drug therapy carefully monitored until no interaction with Lotronex has been established.

Usual Dose

Begin with 1 each day for first 4 weeks. If IBD is not controlled by that dose an increase to 1mg two times a day may be considered.
