Flupirtine Maleate (Katadolon, Metanor, Trancopal Dolo)
Purpose of Medication and Method of Action
Flupirtine maleate is a centrally acting analgesic mainly used to treat pain. It appears to work by suppressing pain perception transmission to the brain and spinal cord through stimulation of descending monoaminergic pathways. The drug appears to work by affecting noradrenaline. Flupirtine does not reduce inflammation or fever. The drug works best for pain associated with noninflammatory conditions. In addition to alleviating pain, flupirtine relaxes muscles through GABA-ergic mechanisms (it inhibits relexes in the spinal cord) and works against seizures. Flupirtine has a low potential for abuse.
Side Effects
Sedation, dizziness, dry mouth, nausea, headache, heart burn, sweating, vomiting.
Drug Interactions
Flupirtine may interact with anticoagulants such as warfarin. A blood test called the PT is needed to monitor this potential interaction. Alcohol or other sedatives such as benzodiazepines can increase the tiredness and dizziness that you may experience with flupirtine.
Cautions and Contraindications
Do not drive or operate heavy machinery until you know how this drug will affect you.
Usual Dose
100 mg three or four times daily by mouth or 150 mg three or four times daily as a rectal suppository. Daily doses of up to 600 mg by mouth or 900 mg rectally have been used where necessary. For children at least 6 years of age, the recommended dose is 50 mg by mouth or 75 mg by rectum three or four times daily to a maximum of 300 mg orally and 450 mg rectally. Use the lowest effective dose for the elderly, patients with kidney or liver impairment, gallbladder disease, or malnourished patients. The duration of treatment should not exceed 4 weeks.
