Celebrex (Celebcoxib)
Purpose of Medication and Method of Action
Celebrex is the first of a new class of anti inflammatory drugs approved for use in both osteo and rheumatoid arthritis. It acts by inhibiting the enzyme cyclooxygenase-2 (COX-2), a key step in producing prostaglandins. Prostaglandins are one of the mediators of the inflammatory response.
Side Effects
Abdominal pain, dyspepsia, and headache are among the most common. Although this class of drugs may produce less gastrointestinal bleeding than the NSAIDS (ibuprofen and naproxen).
Drug Interactions
Celebrex may decrease the effectiveness of ACE inhibitors (Example Accupril) and diuretics such as hydrochlorothiazide (HCTZ) which are antihypertensives. Fluconazole (Diflucan) use will increase blood levels of Celebrex by two-fold and a Celebrex dose reduction may be advisable.
Cautions and Contraindications
Celebrex should be used with great caution in those with a previous history of gastric bleeding following NSAID ingestion.
Usual Dose
Osteoarthritis, 100-200 mg daily in one or two divided doses. Rheumatoid arthritis 100-200 mg two times a day.
