Bextra (Valdecoxib)

Purpose of Medication and Method of Action

Bextra is among a new class of anti-inflammatory drugs approved for use in both osteo and rheumatoid arthritis. Also approved for treatment of dysmenorrhea It acts by inhibiting the enzyme cyclooxygenase-2 (COX-2), a key step in producing prostaglandins. Prostaglandins are one of the mediators of the inflammatory response.

Side Effects

Abdominal pain, dyspepsia, and headache are among the most common. Although this class of drugs may produce less gastrointestinal bleeding than the NSAIDS (ibuprofen and naproxen) it still may occur and if severe gastrointestinal pain should occur or tarry stools, please contact your physician immediately.

Drug Interactions

Bextra may decrease the effectiveness of ACE inhibitors (Example Accupril) and diuretics such as hydrochlorothiazide (HCTZ) which are antihypertensives. Fluconazole (Diflucan) use will increase blood levels of Bextra by two-fold and a Bextra dose reduction may be advisable.

Cautions and Contraindications

Bextra should be used with great caution in those with a previous history of gastric bleeding following NSAID ingestion.

Usual Dose

Osteoarthritis 10 mg (1 tab) each day. Dysmenorrhea - 20 mg (1 20 mg tab) once daily.
