Accolate (Zafirlukast)
Purpose of Medication and Method of Action
Accolate is indicated to prevent the onset of acute asthmatic attacks. It acts by blocking the receptors for leukotrienes, biochemicals produced as a result of an inflammatory reaction. The leukotrienes are believed to be inportant in a cascade of events which lead to bronchoconstriction and an acute asthma attack.
Side Effects
The most common side effects are: headache (13%), infection (4%), nausea (3%), and diarrhea (3%).
Drug Interactions
Accolate can interfere with the metabolism of the anticoagulant warfarin increasing thus increasing anticoagulant activity. A dose adjustment downward of the warfarin may be necessary. The addition to therapy of theophylline or erythromycin to existing Accolate therapy may require an increase in Accolate dosage. Aspirin (ASA) may increase the blood levels of Accolate and increase side effects.
Cautions and Contraindications
Accolate is for prevention of asthma and is of little value in treating acute asthmatic attacks.
Usual Dose
12 years and older - 20 mg two times a day. Children 7-11, 10 mg two times a day.
